Eligibility and Coverage


  Individuals from 18-70 years old may be covered.

  Children from 0-17 years old may be covered.

  Coverage includes:

- Medical Benefit for room and board, special hospital services, surgical expenses, out-patient, In-hospital physician’s visit, ambulance service, emergency treatment in foreign territory and free health check.

- Up to PhP 100,000 Death Benefit.

- Up to PhP 100,000 Accidental death and dismemberment benefit.

  One-time-payment with one (1) year coverage.

 The benefits of this policy shall be paid on top or in excess of the PhilHealth coverage and/or other insurance policies.

Guaranteed Coverage

Room, Board and General Nursing Services Daily Benefit

Pays for the customary charges made by the hospital for room and board, food and general nursing services except special nursing services. Payment for Room and Board is determined by multiplying the room and board daily rate by the number of days for which the insured was hospitalized.

Accidental Death and Dismemberment Benefit

Pays the percentage of the amount of the insurance for sustained accidental injuries within one hundred eighty (180) days after the date of accident.

Death Benefit


Pays the full amount of the insurance upon death of the Insured.

Emergency Treatment in Foreign Territory

Pays for medical expenses if Insured suffers from any medically necessary treatment for emergency situation, occurring during the Insured's short travel outside the Philippines.

Outpatient Benefit and Ambulance Service Benefit


Pays for emergency treatment, and/or emergency dose of anti-rabies, anti-venom, and/or anti-tetanus in a hospital, even if Insured is not admitted. Pays for unlimited medical consultations and administration of vaccine. Pays for the expenses incurred by the Insured on emergency ambulance service.

Special Hospital Services Benefit

Pays for the prescribed medical supplies and services furnished by the hospital such as the use of operating room, treatment room, doctor's fee, laboratory/diagnostic examinations and all other expenses.

In-Hospital Physician’s Visit Benefit

Pays for medical expenses incurred if Insured, registered as an interned patient in a hospital, receives necessary professional medical treatment from a legally qualified Physician.

Surgical Expenses Benefit


Pays for the expenses incurred in a surgical operation, even if no hospital confinement is involved, provided that a legally qualified surgeon performs the surgery.

Free Health Check as Partial Claim Bonus

The Insured Member shall be entitled to Free Health Check thirty (30) days after the end of each policy year if the total claims for the policy year is less than fifty (50) percent of the annual premium paid.


 

Note: Availment of hospitalization reimbursement for the MyLife+ benefit has a waiting period of seven (7) days from the effective date of this Policy or the Insured's coverage, whichever is later. There will be no coverage during the seven (7) - day waiting period.

How to File a Claim

STEP 1: Download and accomplish the required Claim Forms.

STEP 2: Submit the scanned copies of the complete requirements to claimsreimbursement@etiqa.com.ph.

STEP 3: Wait for the approval and release of your claim within 15 working days after submission of complete requirements.
